I can add my add-in and then when I shut down excel and start it up it will still be there and ticked. However if I untick it then shut down and restart excel my add-in is no longer appearing in the list. I have been playing with regedit and suspect that might have done something bad, though I cant see anything I have modifed being different from before.
Check if un-checking it is disabling the Add-in and adding it to the disabled items list. If yes, re-enable them. Follow the steps as shown in the following Microsoft article to repair the Office installation on your computer.
